q       COPY OF CURRENT LICENSE AND INSURANCE CERTIFICATE

             (General Liability, Vehicles and Workman’s comp -Name City as CO-    INSURED)

q       RELEASE FORM  -  signed and notarized

q       PROOF OF LETTERS TO ADJOINING PROPERTY OWNERS

             (Certified Mail receipts and a sample of the letter sent)

q       FIRE HYDRANT PERMIT FROM FIRE DEPARTMENT FOR DUST CONTROL-
or a letter on Company letter head stating how dust will be controlled.

q       LETTER FROM ALL UTILITIES OR PERMIT APPLICATION SIGNED   BY UTILITIES

q       DUMPING LOCATION AND VERIFICATION OF ACCEPTANCE OF DEMOLITION DEBRIS (Letter from dump location)

q       COPY OF CONTRACT

q       PUBLIC WORKS STREET OPENING PERMIT OR APPLICATION SIGNED BY PUBLIC WORKS, CITY HALL -ROOM 222.

q       CORRECT BUILDING STREET NUMBER

q       NUMBER OF UNITS IF BUILDING IS A DWELLING

q       File 90 Day Demolition Delay Form with LUCR: 
William Minor City Hall, 45 Lyon Terr. Room 212, Phone 576-7222

            NOTE* POSSIBLE ENFORCEMENT OF NINETY (90) DAY DEMOLITION

q       RODENT FREE CERTIFICATE FROM HEALTH DEPARTMENT 576-8250

Valid for 30 days from date of issuance.
